Back-to-School Checklist: Vaccinations for 2011-12

Michigan changed its immunization requirements for private and public schools in 2010. We've compiled a list of the new requirements and local clinics where these vaccinations are available.

For Shelby Township and Utica's public and private schools, the first day of the 2011-12 academic year is less than a month away, meaning students have only a few weeks left to bring their immunizations up to date.

In September 2010, the State of Michigan passed new school immunization rules for children entering sixth grade as well as all students 11-18 years of age changing school districts.

Immunization Requirements for Childcare/Preschool Attendance

Birth-1 month

  • None

2-3 months

  • Diphtheria, Tetanus, Pertussis – one dose DTaP
  • Pneumocococcal Conjugate PCV7 and/or PCV13 – one dose
  • H. influenzae type b – one dose
  • Polio – one dose
  • Hepatitis B – one dose

4-5 months

  • Diphtheria, Tetanus, Pertussis – 2 doses DTaP
  • Pneumocococcal Conjugate PCV7 and/or PCV13 – two doses
  • H. influenzae type b – two doses
  • Polio – two doses
  • Hepatitis B – two doses

6-15 months

  • Diphtheria, Tetanus, Pertussis – three doses DTaP
  • Pneumocococcal Conjugate PCV7 and/or PCV13 – three doses
  • H. influenzae type b – two doses
  • Polio – two doses
  • Hepatitis B – two doses

16-18 months

  • Diphtheria, Tetanus, Pertussis – three doses DTaP
  • Pneumocococcal Conjugate PCV7 and/or PCV13 – four doses or age-appropriate complete series
  • H. influenzae type b – one dose on or after 15 months of age or age-appropriate complete series
  • Polio – two doses
  • Measles, Mumps, Rubella – one dose on or after 12 months of age
  • Hepatitis B – two doses
  • Varicella (Chickenpox) – one dose on or after 12 months of age or current lab immunity or reliable history of disease

19 months-4 years

  • Diphtheria, Tetanus, Pertussis – four doses DTaP
  • Pneumocococcal Conjugate PCV7 and/or PCV13 – one dose on or after 24 months or age-appropriate complete series
  • H. influenzae type b – one dose on or after 15 months of age or age-appropriate complete series
  • Polio – three doses
  • Measles, Mumps, Rubella – one dose on or after 12 months of age
  • Hepatitis B – three doses;
  • Varicella (Chickenpox) – one dose on or after 12 months of age or current lab immunity or reliable history of disease

Immunization Requirements for All Public & Nonpublic Schools

4-6 years

  • Diphtheria, Tetanus, Pertussis – four doses DTP or DTaP, one dose must be on or after age 4
  • Polio – four doses, if dose three administered on or after age 4, only three doses are required
  • Measles, Mumps, Rubella – two doses on or after 12 months of age
  • Hepatitis B – three doses
  • Meningococcal – None
  • Varicella (Chickenpox) – two doses of varicella vaccine at or after 12 months of age or current lab immunity or reliable history of disease

7-18 years, including all sixth grade students

  • Diphtheria, Tetanus, Pertussis – four doses D and T or three doses Td if the first was given on or after age 7. One dose of Tdap for children age 11-18 if it has been five years since the last dose of tetanus/diphtheria vaccine was given.
  • Polio – three doses
  • Measles, Mumps, Rubella – two doses on or after 12 months of age
  • Hepatitis B – three doses
  • Meningococcal – one dose for children age 11 or older upon entry into sixth grade or higher
  • Varicella (Chickenpox) – two doses of varicella vaccine at or after 12 months of age or current lab immunity or reliable history of disease

Wed: 8:30 a.m.-6:30 p.m.

A parent or guardian must be available to sign clinic health forms for each child. Bring immunization records and insurance cards.

There are charges for the administration of vaccines–cash or check only. Medicaid/Medicare will be billed for approved vaccines.

MinuteClinic

These vaccinations are available at MinuteClinic, a walk-in clinic located inside the at 25 Mile and Shelby Road.
